SKDI 2012 menempatkan Peroneal palsy pada tabel SISTEM SARAF, subkelompok Penyakit Neuromuskular dan Neuropati, butir nomor 66, dengan Tingkat Kemampuan 3A dan tanpa kata pembatas. Tingkat 3A berarti dokter mampu membuat diagnosis klinis, memberikan terapi pendahuluan pada keadaan yang bukan gawat darurat, menentukan rujukan yang paling tepat, dan menindaklanjuti sesudah pasien kembali dari rujukan. Tetangganya dalam subkelompok yang sama: butir 62 Sindrom Horner tingkat 2, butir 63 Carpal tunnel syndrome 3A, butir 64 Tarsal tunnel syndrome 3A, butir 65 Neuropati 3A, butir 67 Guillain Barre syndrome 3B, dan butir 68 Miastenia gravis 3B. Perbedaan antara 3A dan 3B pada dua butir terakhir itu adalah perbedaan yang perlu dipegang di depan pasien dengan kaki jatuh: kelemahan satu kaki karena satu saraf tepi bukan kegawatan, sedangkan kelemahan yang naik dari kedua kaki adalah kegawatan. Tidak ada pedoman nasional Indonesia yang memuat entri untuk peroneal palsy sebagai penyakit tersendiri. Yang ada adalah dua sumber yang menyebut kelumpuhannya secara langsung. PNPK Kusta menguraikan kerusakan N. poplitea lateralis yang menimbulkan kaki gantung dan kelemahan otot peroneus, dan menyebut nervus peroneus di bawah kaput fibula sebagai tempat penekanan yang dapat berakhir dengan drop foot permanen. Panduan Keterampilan Klinis KMK 1186/2022 menyebut bahwa kondisi yang berhubungan dengan nervus peroneal dapat menyebabkan drop foot, dan menguraikan pola langkahnya. Keterampilan yang diperlukan seluruhnya 4A pada SKDI: inspeksi cara berjalan butir 26, penilaian kekuatan otot butir 25, penilaian sensasi nyeri butir 33 dan sensasi raba halus butir 35, serta refleks tendon butir 46 pada tabel keterampilan Sistem Saraf.SKDI 2012 places Peroneal palsy in the SISTEM SARAF table, subgroup Penyakit Neuromuskular dan Neuropati, item 66, at competency 3A with no qualifying words. Level 3A means the doctor makes the clinical diagnosis, gives initial treatment in a situation that is not an emergency, decides the most appropriate referral, and follows the patient up afterwards. Its neighbours in the same subgroup: item 62 Sindrom Horner at level 2, item 63 Carpal tunnel syndrome at 3A, item 64 Tarsal tunnel syndrome at 3A, item 65 Neuropati at 3A, item 67 Guillain Barre syndrome at 3B, and item 68 Miastenia gravis at 3B. The step from 3A to 3B in those last two is the distinction to hold on to in front of a patient with a dropped foot: weakness of one foot from one peripheral nerve is not an emergency, while weakness ascending from both feet is. No Indonesian national guideline carries an entry for peroneal palsy as a disease in its own right. What exists is two sources that name the palsy directly. The national leprosy guideline sets out damage to the lateral popliteal nerve producing a dropped foot and weakness of the peroneal muscles, and names the peroneal nerve below the head of the fibula as a compression site that can end in permanent drop foot. The clinical skills annex of KMK 1186/2022 states that conditions involving the peroneal nerve can cause drop foot, and describes the resulting gait. The skills needed are all 4A in SKDI: gait inspection item 26, muscle strength testing item 25, pain sensation item 33 and light touch item 35, and tendon reflexes item 46 in the nervous system skills table.

Peroneal palsy adalah kelumpuhan nervus peroneus komunis, saraf yang mengurus pengangkatan telapak kaki. Akibatnya satu, dan terlihat dari ujung ruangan: kaki gantung, atau drop foot 1 2.

PNPK Kusta menguraikan kerusakan saraf ini dengan lengkap, dengan nama lamanya, yaitu N. poplitea lateralis 1:

  • Gangguan sensorik berupa anestesi tungkai bawah bagian lateral dan dorsum pedis.
  • Gangguan motorik berupa kaki gantung dan kelemahan otot peroneus.

Yang membuat saraf ini mudah rusak adalah letaknya. Nervus peroneus melingkari leher fibula tepat di bawah kulit, tanpa otot maupun lemak yang melindunginya, sehingga tekanan dari luar saja sudah cukup. PNPK Kusta menyebutnya sebagai contoh nyata: bidai yang dipasang tidak tepat dapat menekan nervus peroneus di bawah kaput fibula dan mengakibatkan drop foot permanen 1.

Panduan Keterampilan Klinis KMK 1186/2022 memberi cara mengenalinya dari cara berjalan. Pada drop foot, bagian kaki yang lebih dulu menyentuh lantai adalah jari kaki, diikuti telapak kaki, dan tumit paling akhir 2. Urutan itu terbalik dari langkah normal, dan cukup diamati saat pasien berjalan melintasi ruangan.

Yang menaikkan kecurigaan:

  • Penekanan dari luar di sisi lateral lutut: bidai, gips, perban yang ketat, berbaring lama pada satu sisi, atau posisi saat operasi 1.
  • Kebiasaan jongkok lama, bersila lama, atau menyilangkan kaki.
  • Penurunan berat badan yang cepat, yang menghilangkan bantalan lemak di sekitar leher fibula.
  • Trauma atau fraktur di sekitar lutut dan fibula.
  • Kusta, yang di Indonesia merupakan penyebab kerusakan nervus poplitea lateralis yang harus disingkirkan lebih dulu; saraf ini termasuk yang paling sering terkena setelah nervus ulnaris 1.
  • Diabetes dan penyakit sistemik lain yang menimbulkan neuropati.

Kompetensi dokter layanan primer 3A 4. Tugasnya menegakkan diagnosis klinis, memisahkan lesi satu saraf dari lesi radiks maupun lesi medula spinalis, menghentikan penekanan yang masih berlangsung, mencegah kontraktur, lalu merujuk.

Peroneal palsy is a palsy of the common peroneal nerve, the nerve that lifts the foot. Its consequence is single, and visible from across the room: a dropped foot, drop foot 1 2.

The national leprosy guideline sets out the damage to this nerve in full, under its older name, the lateral popliteal nerve 1:

  • Sensory disturbance in the form of anaesthesia of the lateral lower leg and the dorsum of the foot.
  • Motor disturbance in the form of a dropped foot and weakness of the peroneal muscles.

What makes the nerve so easy to damage is where it lies. The peroneal nerve winds around the neck of the fibula directly under the skin, with no muscle or fat over it, so external pressure alone is enough. The leprosy guideline gives the concrete example: a badly fitted splint can compress the peroneal nerve below the head of the fibula and cause a permanent drop foot 1.

The clinical skills annex of KMK 1186/2022 gives the way to recognise it from the gait. In drop foot the part of the foot that touches the floor first is the toes, then the sole, and the heel last 2. That order is the reverse of a normal step, and watching the patient walk across the room is enough to see it.

What raises suspicion:

  • External pressure on the lateral side of the knee: a splint, a cast, a tight bandage, long periods lying on one side, or positioning during surgery 1.
  • Habitual prolonged squatting, sitting cross-legged, or crossing the legs.
  • Rapid weight loss, which removes the fat padding around the neck of the fibula.
  • Trauma or fracture around the knee and fibula.
  • Leprosy, which in Indonesia is a cause of lateral popliteal nerve damage that has to be excluded first; this nerve is among those most often affected, second only to the ulnar 1.
  • Diabetes and other systemic disease that causes neuropathy.

Primary care competency is 3A 4. The task is to make the clinical diagnosis, separate a single nerve lesion from a root lesion and from a spinal cord lesion, stop any compression that is still happening, prevent contracture, then refer.

  • Riwayat operasi dan posisi selama operasi.
  • Trauma pada lutut atau tungkai bawah.
  • Penurunan berat badan yang cepat.
  • Riwayat kusta pada pasien atau kontak serumah, dan riwayat bercak kulit yang mati rasa 1.
  • Nyeri punggung bawah yang mendahului, dan apakah nyerinya menjalar ke tungkai.
  • Diabetes dan penyakit sistemik lain.
  • Riwayat tersandung dan jatuh, karena itulah bahaya sehari-harinya.

What makes the diagnosis. One foot that cannot be lifted, with sensory loss confined to the lateral lower leg and the dorsum of the foot, no low back pain and no bladder disturbance 1.

History:

  • When the foot started to drop, and whether it came on suddenly or gradually.
  • What pressed on the outer side of the knee before the symptoms began: a splint, a cast, a tight bandage, long periods lying on one side, prolonged squatting, sitting cross-legged, or crossing the legs 1.
  • Any surgery and the positioning during it.
  • Trauma to the knee or lower leg.
  • Rapid weight loss.
  • Leprosy in the patient or a household contact, and any history of a numb skin patch 1.
  • Preceding low back pain, and whether it radiated down the leg.
  • Diabetes and other systemic disease.
  • A history of tripping and falling, because that is the day to day danger.
